The first half finished goalless before Millwall struck seven minutes after the restart. Cundle found the breakthrough at 52'00'', giving the visitors control and changing the tone of the tie. QPR answered with a cluster of substitutions, sending on Kwame Poku and Jonathan Varane just after the hour, then introducing Alfie Lloyd before the 70-minute mark as they chased a route back into the contest.
Millwall also reshaped their side in the closing stages. Tairyk Arconte and Kyrell Lisbie came on at 75', Mark Sykes followed at 76'57'', and further changes arrived through Raees Bangura-Williams and Daniel Kelly in the final minutes. The equaliser finally came at 85'49'', when Sykes diverted the ball into his own net to make it 1-1 and send the cup tie to penalties.
From the spot, Millwall held their nerve better. Cooper and Sykes both converted, while QPR missed through Chair, Harvey Vale, Koki Saito and Poku. Camiel Neghli and Arconte also failed for Millwall, but the visitors still carried the result. It was a messy finish, yet the late swing and shootout drama supplied the key highlights in a sharp summary of a cup night decided by fine margins.
